Schedule your COVID-19 Vaccine (Moderna or Pfizer) at the Robert E. Bush Naval Hospital, Clinic D

Beginning 20Oct2021, NMRTC Twentynine Palms will be offering the Pfizer COVID-19 vaccine to eligible beneficiaries for ages 12 years and older in Clinic D, Monday Through Friday from 8-16:00. Walk-ins are welcome based on availability or appointments can be made by using the DHA appointing portal at https://informatics-stage.health.mil/COVAX or contact 760-830-2698. The Moderna COVID-19 vaccine continues to be available in Clinic D for ages 18 and older.

**For those interested in obtaining the Johnson and Johnson COVID vaccine, please utilize vaccines.gov or myturn.ca.gov to find a site in the local area.**

ELIGIBILITY:

• TRICARE beneficiaries

• DoD civilian employees

• DoD contractors

• Active duty members and reservists

THE FOLLOWING MAY PREVENT YOU FROM RECEIVING THE VACCINE:

• Having received another vaccine within 14 days of your COVID-19 vaccine appointment

• History of anaphylaxis to other vaccines or COVID-19 vaccine components

• Being in isolation with COVID-19 or in quarantine after exposure to COVID-19

• If you will not be in the area to receive your second dose

• Scheduling your second dose before the recommended window (earlier than 28 days after your first dose of Moderna & earlier than 21 days after your first dose of Pfizer)

SECOND DOSES:

• Please bring your COVID-19 Vaccination Record Card when returning for your second dose. Reference this card when scheduling your 2nd dose.

• If you are attempting to book an appointment for your 2nd dose but are unable to find available appointments, contact (760) 830-2698 for scheduling assistance.

ACKNOWLEDGEMENT

1. I am currently eligible for the COVID-19 vaccine based on the information listed above.

2. If I am not eligible and continue to make an appointment, I understand that I will be turned away at my appointment.
