For booster shot, you must be over 18+ and at least 6 months out from your second dose of Pfizer or Moderna, or at least 2 months out from J&J

Appointment is required. No walk-ins accepted.

Vaccine Eligible Groups

The 61st MDS is currently vaccinating all phases; which includes all the following beneficiaries age 12 and up.
Eligible Personnel
Active Duty Members and Reservists (incl. National Guard)
All TRICARE Beneficiaries (dependents, retirees etc.)
DoD Civilian Employees and Contractors
Family members of DoD Civilians and Contractors are NOT eligible to receive the vaccine at this facility

I am currently eligible for the COVID-19 vaccine based on the information displayed above for eligible patient groups.

I have reviewed the Pfizer COVID-19 Vaccine Fact Sheet at https://www.fda.gov/media/144414/download Please contact your Primary Care Manager for any questions or concerns.

I will NOT come to the appointment if I have been instructed to quarantine or I am experiencing COVID-19 symptoms (fever, cough, difficulty breathing, fever, chills, muscle aches, fatigue, headache, sore throat, stuffy nose, nausea, or sudden loss of taste and smell). Contact your primary care provider.

Booking Instructions:

  • Appointments will be available on those days that are highlighted in blue
  • Click on the actual date (number), instead of the blue box
  • Include your phone number AND e-mail address in case appointments need to be re-scheduled.
  • Use slashes for your birthdate and dashes for your phone number

  • Appointment Information:

  • You will need your military ID/CAC or government issued identification.
  • COVID-19 safety precautions, including social distancing and wearing an appropriate mask, will be enforced
  • 2nd Dose: You should get your second shot after 21 days. However, your second dose may be given up to 42 days after the first dose, if necessary. You should NOT get the second dose early. Please book appointment for second dose as soon as you have received the first dose.
