Welcome to the 377th MDG COVID-19 Vaccination Appointment Portal

We are proud to serve you!

MODERNA vaccine is available to ages 18+. PFIZER vaccine is available for ages 12+. COVID-19 vaccines are available for Active Duty members, AD dependents, 377 MDG beneficiaries, and CAC carrying employees of Kirtland AFB. COVID-19 vaccines are only being offered on Fridays.

CDC recommends that the following groups should receive a booster dose of Pfizer-BioNTech/Comirnaty and Moderna COVID-19 vaccines at least 28 days after completing dose 1 and 2:

*Certain immunocompromised people (i.e., people who have undergone solid organ transplantation or have been diagnosed with conditions that are considered to have an equivalent level of immunocompromise).

CDC recommends that the following groups should receive a booster dose of Pfizer-BioNTech/Comirnaty and Moderna COVID-19 vaccines at least 6 months after completing a previous series:

*Age 65 years and older

*Age 18+ who live in long-term care settings (see link for examples: https://www.cdc.gov/coronavirus/2019-ncov/vaccines/booster-shot.html#long-term-care)

*Age 18+ who have underlying medical conditions (see link for examples: https://www.cdc.gov/coronavirus/2019-ncov/need-extra-precautions/people-with-medical-conditions.html)

*Age 18+ who who work or live in high-risk settings (see link for examples: https://www.cdc.gov/coronavirus/2019-ncov/vaccines/booster-shot.html#HighRisk)

Eligible individuals may choose which vaccine they receive as a booster dose. Some people may have a preference for the vaccine type that they originally received (Pfizer BioNTech, Moderna, or Johnson and Johnson/Janssen vaccine), and others may prefer to get a different booster. CDC’s recommendations now allow for this type of mix and match dosing for booster shots.

This application is for scheduling COVID-19 vaccinations only. If you are a beneficiary of the 377th MDG and need other care, please call the MTF at 505-846-3200. If this is a medical emergency, please call 911 or report to the nearest Emergency Room.
