COVID-19 VACCINES ARE NOT AVAILABLE BY WALK-IN. BY APPOINTMENT ONLY!

NMRTU Key West currently administers the Moderna AND Pfizer vaccine, which requires 2 doses.

MODERNA: Dose 2 should be scheduled at 28 days following Dose 1 and is provided on WEDNESDAYs ONLY.

PFIZER: Dose 2 should be scheduled at 21 days following Dose 1 and is provided on FRIDAYs ONLY.

Acknowledgements:

1. I am a TRICARE eligible beneficiary, DoD employee, or DoD contractor.

2. I am currently 18+ years of age for MODERNA or 12+ years of age for PFIZER.

3. I fully understand Moderna is offered on Wednesdays and Pfizer is offered on Fridays ONLY.

4. If I am not currently eligible OR I make an appointment on the wrong day, I understand that I may not be assisted further at my appointment.

*If you are IMMUNOCOMPROMISED and scheduling your Additional Dose, please use the 'COVID-19 Additional Dose' option to schedule your vaccination. The Additional Dose must be at least 28 days after the second dose. The Additional Dose is NOT authorized for anyone who received the Janssen (J&J) vaccine! Please bring your original vaccination card with you! More info on the Additional Dose for Immunocompromised are available on the CDC website at https://www.cdc.gov/coronavirus/2019-ncov/vaccines/recommendations/immuno.html.

**Pfizer Booster Shots are available, please use the 'COVID-19 Additional Dose' option to schedule your vaccination. The Pfizer Booster Shot must be at least 6 months after the second dose. The Booster shot is NOT authorized for anyone who have received the Janssen (J&J) OR Moderna vaccine! All patients seeking Pfizer Booster Shots should review CDC guidelines and meet the criteria for Pfizer Booster Shots (https://www.cdc.gov/coronavirus/2019-ncov/vaccines/booster-shot.html). Please bring your original vaccination card with you!

Active Duty: If you have received COVID vaccine at a local, non-DoD vaccine provider, please bring proof of vaccine to the clinic ASAP! Also, if you wish to DECLINE the vaccine for any reason (religious, medical, or outright refusal), please schedule yourself, as either Initial or Second dose, into one of the available time slots and a provider will be available for counseling.
