***All members on FLIGHT ORDERS willl be DNIF'd for 48 hours after receiving the vaccine*** ***DO NOT SCHEDULE A VACCINE IF YOU ARE UNDER THE AGE OF 18*** Each member is asked to review the Emergency Use Authorization for the Moderna COVID-19 Vaccine, located at https://www.modernatx.com/covid19vaccine-eua/recipients/. Each member is asked to watch the Public Health COVID-19 vaccination video prior to arriving for their vaccination, located at https://www.columbus.af.mil/News/Video/. Each member is asked to review and complete the DHA FORM 207 COVID-19 VACCINE SCREENING AND IMMUNIZATION DOCUMENTATION, located at https://health.mil/Reference-Center/Forms/2021/03/25/DHA-Form-207-COVID-19-Vaccine-Screening-and-Immunization-Document%20%3C%20Caution-https://health.mil/Reference-Center/Forms/2021/03/25/DHA-Form-207-COVID-19-Vaccine-Screening-and-Immunization-Document%20%3E, prior to arrival, and bring the signed copy with them to their appointment. The consent form is for those receiving the first dose only. Members who have questions for the provider or have yes responses will be directed to speak with a provider. Members that arrive without their consent forms completed may experience delays. Members reporting for their second dose are asked to bring their CV-19 shot card with them as this will be required for documentation. Each member is asked to arrive 5 minutes prior to appointment with their DoDID and COVID-19 shot card readily available to ensure we can efficiently distribute the vaccine.
