The COVID-19 Vaccine Clinic is open Monday through Friday from 7am to 3pm
Closed weekends and federal holidays.


Please read the statements below prior to booking your appointment.

1. I am an Active duty, National Guard, or Select Reserve Service Member, a TRICARE beneficiary age 12 and up, a DoD employee, or a select DoD contractor who usually receives the influenza vaccine as part of a DoD occupational health program.

2. I confirm that I am age years 12 and up. The vaccine cannot be administered to persons under the age of 12. A parent/guardian must accompany patients under 18 years old.

3. I understand that FBCH is currently only administering the Pfizer COVID-19 vaccine. The Pfizer COVID-19 vaccine can only be administered to persons aged 12 and up. This vaccine is given in a two-dose series separated by 21 days.

4. I understand that FBCH is only offering the Pfizer vaccine. If you received the Moderna vaccine for you 1st dose, you are not eligible to get your 2nd vaccine at FBCH. We only offer the Pfizer vaccine.

5. I understand that if I am not currently eligible based on the above criteria and continue to make an appointment, that I will be turned away at my appointment.

Booster doses are now being offered for select groups. People age 65 and older, residents in long term care facilities, people age 18-64 with underlying health conditions, and people 18-64 who are at increased risk due to occupational or institutional setting are eligible for a booster dose 6 months after their 2nd Pfizer dose. Please speak with your Primary Care Provider with any questions or concerns. For those who are moderately to severely immunocompromised, a 3rd dose is available 28 days from your 2nd dose of the Pfizer vaccine. Please check back for updates.

Individuals must self-identify as requiring the additional third dose. A provider referral or script is NOT required in order to receive the additional third dose. Individuals are encouraged to speak with their Primary Care Provider if they are unsure about their current health status. Patient must wait at least 28-days after completing their 2-dose mRNA vaccine series prior to receiving the additional third dose. For people who received either Pfizer or Moderna mRNA vaccines, a third dose of the same vaccine should be used. This third dose recommendation DOES NOT apply to individuals who received the Janssen/J&J vaccine. FBCH currently offers the Pfizer mRNA Covid-19 vaccine only.
