Naval Medical Center San Diego is now offering Pfizer-BioNTech COVID-19 vaccine for 12-15 year olds. Please read through the below information to continue making an appointment. 

  • Coronavirus Disease 2019 (COVID-19) is a respiratory illness that is easily spread from person to person and can be deadly. It has caused a world-wide pandemic and a public health emergency in the U.S. military and throughout the United States.
  • Based on the public health emergency, the U.S. Food and Drug Administration has issued an “Emergency Use Authorization” for a newly developed vaccine to prevent COVID-19 in people 12-years-of-age and older. You are being offered that vaccine.
  • The top officers and enlisted leaders of all the Armed Forces and the top military doctors all agree with the recommendation of the U.S. Center for Disease Control and Prevention that eligible people get vaccinated now.
  • Although it is recommended, receipt of this vaccine is voluntary for all Americans, including members of the Armed Forces.

By Proceeding You As A Patient Or Parent Acknowledge The Following: 


1. As of April 12th, 2021 I am currently eligible for the PFIZER COVID-19 vaccine based on the information listed below. 
  • All TRICARE beneficiaries over the age of 12.
  • All DoD civilian and contract employees with appropriate identification. (ex. CAC Card)


2. PARENTS: All of our locations should be treated like medical clinics. We have medical staff working with sharp objects and sensitive vaccine materials. Children are welcome to accompany you to your appointment, but they cannot be allowed to roam freely as it puts you, the child, and our medical staff at risk. 


3. If scheduling my first dose, I am available to return to Naval Medical Center San Diego vaccine location in 21-28 days to receive my second dose of Pfizer vaccine. Please do not schedule your second dose until after receiving your first dose. 


4. If you have received a first dose of Moderna, please do not book an appointment at this location for your second dose. If you are active duty and received your first dose of Moderna in another geographic area, please coordinate through your unit's medical point of contact for additional information. 5. In order to shorten the length of your visit for your vaccination, please print and fill out THIS FORM prior to your arrival. (Note: form must be downloaded and opened on a desktop computer) 


Walk-ins welcome, however appointments will receive priority and are encouraged to ensure enough vaccine supply is available.
